    Hang Tag Information:

    When filling out a hang tag, please provide the follow:

    • Serial number
    • Description of the problem
    • SV number (if applicable)
    • Date

    Return of Warranty Parts:

    When replacing a part under warranty please attach a hang tag and return the part along with a copy of the claim to the following shipping address:
